This privacy policy explains how I use any personal information I collect about you.
I will collect information about you after our first session together. I will ask you to complete an online client contact form. This includes personal details, emergency contact details and GP details. I also keep brief written session notes. I also keep an attendance record of sessions and any payments made. This is held on a secure practice management system called WriteUpp.
I collect personal information about you to facilitate contact with you.
If your health is in jeopardy (provided I have your consent) I may share your contact information with an emergency healthcare service (e.g. GP, Mental Health Crisis Team). In case of an emergency when you are with me, I may also contact your emergency contact. I may also pass this information to the emergency services if required.
If I have become aware of your intent to cause harm to another person/organisation (e.g. terrorism), the law may require that I inform an authority without seeking your permission. In such a situation, the law may require that I share your personal information without your knowledge.
Session notes are used for my own practice. I am required to attend regular supervision with a supervisor who is trained and qualified in supervision. This is in order to ensure that you receive the best possible service. Any discussions I have in supervision would remain anonymous.
I also record attendances and payments in a database for financial purposes such as my annual tax return.
You have the right to request a copy of the information that I hold about you. If you would like a copy of some or all of your personal information, please email or write to us at the address below. I want to make sure that your personal information is accurate and up to date. You may ask me to correct or remove information you think is inaccurate. If any information changes please let me know.
I will hold onto your written information for up to seven years past the end of our working together. This is so that I have a reference of our work in situations such as you returning to counselling in the future. After this time has passed, I will shred the written information.
I keep my privacy policy under regular review and I will place any updates on this web page. This privacy policy was last reviewed on 18th Sept 2024.
